Your first visit will be an initial consultation and oral examination.

You will need to bring the following with you that day:

1. Referral slip from your general dentist (if you have been referred by a general dentist)
2. Any x-rays that have been taken in the last year.
3. Your dental and medical insurance card.
4. List of medications currently being taken.  

About x-rays: Please bring any x-rays that you have had taken by your general dentist or physician within the past year to your appointment. You may also request for your general dentist to forward your x-rays to our office. Without x-rays. Dr. Cusumano is not able to accurately assess your condition and develop an appropriate treatment plan. If you do not have any x-rays, our facility is equipped to take them during your consultation.

Important: All patients under the age of 18 must be accompanied by a parent/guardian at the consultation visit.
